| Older Families Planning Project |

 OFPP, a partnership between Sheffield Neighbourhoods and
Community Care and Sheffield Care Trust, is aimed at supporting older families
of people with learning difficulties to make plans for the future.
By looking at the health and social needs of the whole family and involving the most important people in the families' lives,
we try to help to come up with realistic and achievable plans for coping in the future and dealing with emergencies.
